Survey Objective

The CAHPS PCMH Survey was developed to address the patient experience and care processes related to patient-centered medical homes (PCMH). The survey includes both pediatric and adult populations that are reported at the practice level.

Survey Detail

The annual CAHPS PCMH survey asks questions important to care measures including:

  • Access
  • Communication
  • Comprehensiveness
  • Self-management support
  • Shared decision making
  • Office staff

Additionally, the survey measures individual items in the following areas:

  • Rating of provider
  • Information
  • Coordination of care

SPH has the ability to conduct the survey using all acceptable modes of data collection including mail, telephone, IVR, internet, and mixed methodologies.
