This survey may be used by NCQA accredited health plans to support the Population Health Management (PHM) Standard 5: Complex Case Management. As part of this standard, NCQA encourages organizations to annually evaluate their members’ experience with case management. SPH’s Case Management Survey also supports PHM Standard 6: PHM Impact, using Case Management Survey results to measure the effectiveness of PHM functions.
