Alpharetta, GA – SPH Analytics, the top ranked vendor for population health and value-based care client satisfaction, announced today that one of its key clients, the Consortium for Southeastern Hypertension Control (COSEHC), will present at HIMSS19. The session will highlight COSEHC’s work as a practice transformation network (PTN) under a cooperative agreement from the Centers for Medicare and Medicaid Services (CMS) Transforming Clinical Practice Initiative (TCPI). The PTN implemented a patient-centric population health improvement program, powered by SPH Analytics’ Population Care patient registry, that would establish health-improvement targets, reduce costs, and help implement sustainable business processes in practices serving the health of millions of persons across nine southeast states.

For more than 20 years, COSEHC has worked to reduce cardiovascular disease, diabetes morbidity and mortality. COSEHC’s experience enabled it to be one of 29 groups awarded with a cooperative agreementunder HHS’ new Transforming Clinical Practice Initiative. CMS funding supports the PTN’s provision of clinical expertise and technical assistance to providers throughout the Southeast.
