T he SPH Analytics Case Management Survey was developed to help healthcare organizations obtain feedback from patients about their experience and satisfaction with their Case Manager and case management services received.

SPH’s Case Management Survey includes approximately 20 questions and may be customized to support your organization’s goals. Survey topics include:

  • Patient referral to the program and follow up
  • Coordination of care
  • Communication
  • Effectiveness of case management services
  • Helpfulness of information and written materials
  • Overall satisfaction with case management and with their Case Manager

Supporting NCQA PHM Requirements

This survey may be used by NCQA accredited health plans to support the Population Health Management (PHM) Standard 5: Complex Case Management. As part of this standard, NCQA encourages organizations to annually evaluate their members’ experience with case management. SPH’s Case Management Survey also supports PHM Standard 6: PHM Impact, using Case Management Survey results to measure the effectiveness of  PHM functions.
